Primary Care Transformation Specialist, Senior

Key Role:

Maintain responsibility for overseeing the design, development and application of quality improvement models in clinical settings to transform primary care practices. Lead the implementation of all aspects of practice transformation, including leveraging expertise in quality measures, workforce burden reduction, and Health IT (HIT). Provide technical support to health care providers working on redesign care delivery efforts.

Basic Qualifications:

-7+ years of experience with improving clinical quality, quality measures, or convening action-oriented quality improvement groups

-Experience with implementing practice transformation initiatives designed to help ambulatory care providers achieve health transformation goals in multiple settings, including independent, rural, health system, and academic

-Experience with facilitating large scale learning sessions

-Knowledge of CMS and MACRA legislation

-Knowledge of Medicaid, Medicare and commercial payment environments and structures

-Knowledge of Medicare Data Systems and EHRs

-Ability to obtain a security clearance

-BA or BS degree in Health Sciences, Nursing, Business, or Healthcare related field

Additional Qualifications:                                                                                          

-Experience with Medicare and Medicaid Services

-Ability to discuss health care delivery redesign and approaches

-Ability to work independently and within a fast-paced, and team-oriented environment

-Possession of excellent oral and written communication skills

-MA or MS degree
