Long Term and Post-Acute Care Expert, Lead

Key Role:

Work as a hands-on adviser for clients in leadership roles in the areas of patient safety and quality improvement in long term and Post-Acute Care (LTPAC) settings. Lead projects and initiatives and act as a subject matter expert for project teams. Support analysis, interpretation, and visualization of quality data, causal analysis processes, and use quality improvement methodologies effectively. Maintain responsibility for leading the design, development and application of quality improvement models to transform and improve patient care in LTPAC settings.

Basic Qualifications:

-5+ years of experience with convening action-oriented groups in LTPAC settings

-7+ years of experience with patient safety and quality and performance improvement, including facilitating improvement initiatives using methodologies, including Lean Six Sigma, Design Thinking, FMEA, and root cause analysis

-Experience with facilitating large scale learning sessions

-Knowledge of evidence-based clinical care practices and national benchmarking

-Ability to provide coaching and support of quality improvement activities to achieve improvements in areas, including reductions in avoidable hospital admissions and readmissions and ED use, falls, and pressure ulcers

-Ability to obtain a security clearance

-BA or BS degree

Additional Qualifications:                                                                                      

-Experience with Centers for Medicare & Medicaid Services (CMS)      

-Experience with LTPAC quality measures and quality reporting programs

-Knowledge of post-acute care reimbursement models

-Knowledge of Patient-Driven Payment Model (PDPM) for Skilled Nursing Facilities

-Ability to work independently and within a fast-paced, team-oriented environment

-Possession of excellent oral and written communication skills

-Certified Professional in Healthcare Quality (CPHQ) Certification
