Key Role:
Design and implement approaches to address social determinants of health. Coordinate the improvement efforts of health outcomes and address fragmentation of care for affected beneficiaries. Lead and manage initiatives and support multiple cross disciplinary programs to execute on clinical, payment, and population health interventions to transform healthcare for Medicaid and CHIP-covered beneficiaries affected by the opioid crisis.
Basic Qualifications:
-7+ years of experience with Medicaid and CHIP policies
-Experience in working with community agencies and collaborating across different care settings
-Experience with working across multiple Health and Human Services (HHS) or federal government agencies
-Experience with facilitating large-scale learning and improvement projects
-Ability to support the development and implementation of best practices for population health improvements
-BSW or BA or BS degree in Healthcare Administration, Public Health, or Business
Additional Qualifications:
-Experience with Alternative Payment Models (APMs) or developing new state payment models
-Experience in substance abuse or behavioral health settings
-Experience with CMS
-Ability to foster collaboration and develop professional relationships with multiple stakeholder organizations
-Ability to discuss population health models and approaches is a plus
-Ability to work independently and within a fast-paced, team-oriented environment
-Possession of excellent oral and written communication skills
-MSW, MA, or MS degree a plus
